This is not a good demonstration of a correct injection technique 1) His hand is touching the tips of the needle which increase the risk of infection 2) I did not see the tip of his needle or the flow of the injected material. You want to see the injected fluid flowing nicely to down the suprapatellar recess. 3) Why do you have to jiggle the needle? You have the ultrasound, just find the tip. 4) 30-45 degree knee flexion works the best to increase the fluid in the recess. 90 degree will actually make the fluid go away

I had it without anesthetic and it was painful!

No doctor, using anesthetic to the area of injecting prp does NOT interfere with effectively treating a tendon. By the way, you did not treat the tendon damage sufficiently. You barely treated the thin and frayed tissues at all per the msk u/s. No wonder you call it “relief” rather than regenerative and say that it is better than cortisone, (which destroys healthy tissue). While your patient may have “relief”, the tissue will not regenerate to become fully viable again. Better than steroid injection? You bet but better than prolo/prp, a definite no. In the right hands, using best protocols, this patient would have proper regeneration of all unhealthy tissue rather than some relief of symptoms. Treat cause, treat fully, and treat to sufficiency in alignment with when and how the body heals these tissues.
